Defining the Problem
A lack of access to a patient’s accurate and comprehensive Electronic Health Record (EHR) is a major barrier to proper healthcare and diagnosis. Due to a lack of a universal healthcare database, a patient that is seen at multiple health facilities may have several fragmented records stored in separate systems. Failure to provide an accurate health history is one of the leading causes of misdiagnosis, which is a prevalent problem in the US where 12 million patients are misdiagnosed annually.
